Jessica Simpson's alleged addictions to pills and alcohol have prompted husband Eric Johnson to confront her with an ultimatum: Either she gets help, or he will seek to take legal custody of their children. 

According to Radar Online, Simpson's substance issues have now reached "crisis level," with little sign of abating any time soon, forcing the former NFL quarterback to take drastic measures.

"Things reached a climax in early May, when Eric told Jess he has had enough and issued his ultimatum," a source said. "He is worried sick that Jess' conduct is going to start affecting their two children and that their oldest is starting to realize there's something wrong with mommy."

Simpson's vices are reported to include the prescription drug Adderall, which she reportedly takes to keep her slim and petite figure in order, and scotch, which she allegedly takes to wash down all the pills.

Earlier this month, the Internet was abuzz with speculation the 35-year-old Simpson was wasted during a recent live TV appearance on the Home Shopping Network, where she appeared "agitated and unsteady on her feet" during a two-hour show alongside host Colleen Lopez.

During several segments of the show, Simpson stepped away from the cameras for unexplained reasons, and later, her mother Tina made an appearance. Earlier this year, Tina Simpson was rumored to have tried to convince her daughter to go to rehab, even offering to enroll in a program with her.

Tina Simpson is said to have developed a serious drinking problem as far back as two years ago, when she underwent a messy divorce from Jessica's father Joe.

"She can't be in denial any longer about the toll that her problems have taken on her marriage, or she could lose her husband her children -- and her money," a source added of Jessica.
